Update $metaTablesSQL to use standard USER_CATALOG view name instead of
CAT synonym, and standardize the query across the whole Oracle drivers
family.

Update the $metaColumnsSQL statements in oci8, oci8po, pdo_oci and
odbc_oracle drivers to use USER_TAB_COLUMNS instead of COL. Change was
not applied to the deprecated oracle driver.
Since the contents of the COL.NULLS column (`NULL` / `NOT NULL`) is not
the same as the equivalent USER_TAB_COLUMNS.NULLABLE (`Y` / `N`), we
can get rid of the CASE statement in $metaColumnsSQL2 and simplify the
processing in metaColumns() method, to use a simple string comparison
instead of a strncmp() call to populate ADOFieldObject's not_null
property.

The conflicts resolution applied when merging the Standardized file
headers (commit e9dcce3df24912ad869d0193f0b419f2309101fc) was seriously
messed up, actually overwriting a number of changes in the master
branch.
Rather than trying to go and fix things one by one which has a high risk
of messing things further, it's easier to redo the merge from a clean
slate, so this commit reverts the following:

A non-regexp based function has been introduced to replace a regexp based section of code that has been subject to numerous tweaks as more extreme test cases have appeared. This is now done this like this to help maintainability and avoid the need to rely on regexp experienced maintainers.

The ADOdb_oci8::SelectLimit() method performs Oracle-specific query
optimization, manipulating the SQL to apply hints in a way that is not
compatible with the oci8po driver, due to conversion of query
parameters ('?' vs oci8 native ':xx').
To avoid the problem, we define the SelectLimit() method directly in
ADODB_oci8po, and rely on the slower ADOConnection::SelectLimit()
method from the base class.
Also, to avoid issues with prepared statements causing PHP to throw a
Warning: "oci_execute(): supplied resource is not a valid oci8
statement resource", we retrieve the prepared statement's SQL and pass
it on to ADOConnection::SelectLimit().

The ADOdb Extension's development stopped at version 5.04. It is no
longer maintained or supported, yet the library still contains code
referencing it. If those code branches are executed, ADOdb will not
perform properly, due to missing constant declarations (among other
things).
This commit removes all references to the Extension.

The OCIFetchinto function is causing segfaults on php7 - probably
because the fields array is not initialised or it is optimised out. This
fixes just changes to use the safer function oci_fetch_array instead.
